1. Enter the information
Enter the information electronically on each field. Mandatory fields are marked with an asterisk (*).
2. Submit the information
Submit the information by clicking on the Submit form button at the bottom of the sample form.
3. Sample code
After submission a sample code is generated by the system and a new web page is displayed. Please, read carefully the instructions on the top of that web page. A copy of the submitted form in PDF format is also shown.
4. Write the sample code
Write the sample code in clear CAPITAL LETTERS on the tube (or tubes if there are more than one tube for the same dog). The sample code also appears at the bottom of the generated PDF file along with a bar code.
5. Ship the sample
Ship the sample to Hannes Lohi's lab according to the shipment instructions given in the sample form. If possible please print the PDF copy of the sample form, sign it and enclose it in the same envelope with the corresponding sample tubes. This will help us to speed up the processing of the sample when it arrives to the lab.
